Australian Magellan Observer's Report Form

Note: All observers are requested to fill out the following form (one per observing run). Please be frank in your responses as this information is vital to us in evaluating the operation of the Observatory telescopes, instrumentation, and facilities.

The Australian Gemini Office in partnership with the Las Campanas Observatory aims to provide excellent instrumentation and a high level of observing support and facilities to visiting astronomers. In order to help us maintain this high standard, we ask that you take a few minutes at the end of your observations with the Magellan telescopes to fill in the report form below. Your feedback is vital in enabling us to improve your experience using the Magellan telescopes, which in turn will support the case for continued access into the future.

This report form will be sent automatically to the Carnegie Associate Director for Magellan (Mark Phillips) and to the Australian Gemini Scientist (Stuart Ryder). In addition, members of the Magellan Council and local management of Magellan and Las Campanas Observatory (LCO) will receive a copy.
